NCM300 and Honeywell HE300 humidifier wiring diagram

Hi,
Please provide wiring diagram for NCM-300 for Honeywell HE300 humidifier. Currently it is wired somehow, but having challenges. Humidifier kicks in when humidity level is low, but dampers MA-MD5 are not closing (when humidifier is on). Therefore all humid air is just lying in furnace, nowhere to go. Let me know if you need any additional information.
