Sara, I do not have the panel you mentioned. I have a EWC ST-2D.
I have two dampers for the first floor(#1 and #2) and one damper for the second floor(#3). They are arranged in a "T". #1 duct goes left, #2 duct goes right and #3 comes out the middle leg.

Sara, I could not find the marking you mentioned. Their is a number 4899 stamped above ULTRA-ZONE.

Thanks for agreeing. That's what I did. I couldn't find and markings on the blue cover and I don't have a 1/16" allen wrench so I could look inside. But I'm good for now.
